ESSENTIAL DUTIES AND RESPONSIBILITIES: The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive. Perform medical assessment of patients. Complete home visit Encounter Summary within 48 hours of appointment. Assure compliance with licensing, certification and accrediting bodies. Represent company and the clinic as reasonably required at internal and external meetings or events. Maintain working equipment and supplies. Coordinate patient care with patient's associated home health agency. Review patient case document in a timely manner. Full-time or part-time available.
